50-mL bottle Childn & toddler weighing >10 to ≤33 kg 15 mg/kg (1.5 mL/kg), max of 60 mg/kg daily not exceeding 2 g daily. Max vol per administration: 49.5 mL.
100-mL bottle Adult, adolescent & childn weighing >50 kg & no additional risk factors for hepatotoxicity 1 g (100 mL), max of 4 g daily. Max vol per administration: 100 mL;
>50 kg w/ additional risk factors for hepatotoxicity 1 g (100 mL), max of 3 g daily. Max vol per administration: 100 mL;
>33 to ≤50 kg 15 mg/kg (1.5 mL/kg), max of 60 mg/kg daily not exceeding 3 g daily. Max vol per administration: 75 mL. Min interval between each administration must be at least 4 hr.
Patient w/ severe renal impairment (CrCl ≤30 mL/min) Reduce dose & increase min interval between each administration to 6 hr.
Adult w/ hepatocellular insufficiency, chronic alcoholism, chronic malnutrition (low reserves of hepatic glutathione), dehydration Max daily dose: 3,000 mg.
